Perianal dysplasia refers to abnormal cell changes in the skin around the anus. These changes can range from mild (low-grade) to severe (high-grade). Bowen’s disease is a type of high-grade dysplasia, essentially a precancerous stage. Both are often caused by the Human Papillomavirus (HPV).

These conditions often have no noticeable symptoms. However, some people might experience itching, burning, bleeding, or a change in the appearance of the skin around the anus, such as redness, thickening, or a scaly patch.

Diagnosis typically involves a visual examination of the area and a biopsy, where a small tissue sample is taken for examination under a microscope. Additional tests like an anoscopy or high-resolution anorectal imaging (HRAI) might be used for a more detailed evaluation.

Treatment depends on the severity of the dysplasia. For mild to moderate cases, options might include topical medications like imiquimod cream or watchful waiting with regular monitoring. For high-grade dysplasia or Bowen’s disease, surgical removal becomes more likely.

Several surgical approaches can be used, depending on the location and size of the affected area:

  • Wide Local Excision: Removal of the abnormal tissue with a margin of healthy surrounding tissue.
  • Electrosurgical Excision Procedure (LEEP): Uses a looped electrical current to remove the abnormal tissue.
  • Laser Therapy: A focused laser beam vaporizes the abnormal tissue.

Recovery time varies depending on the type of surgery performed. You might experience some discomfort or pain, which can be managed with medication.
